Permission to Administer Medication

In order for school personnel to administer any prescription medication or over the counter medication on a routine basis to students we must have this form completed and signed by either the student’s parent/guardian or doctor.

Re-Enrollment Forms

These forms must be filled out every year to re-enroll each student for the next school year. They are due by April 1.
